Missing solutions for planning care for seriously ill patients, who after being discharged from hospital are taken care of by relatives, unprepared for their new role. Such families do not receive institutional support to provide appropriate conditions for the recovery of the patient.
How does innovation work?
“THE PROVIDER OF COMPREHENSIVE CARE AT HOME“ is a program for people with multiple ailments and people whose health and daily functioning have deteriorated rapidly. The aim of the project is to support dependent people by assisting relatives/caregivers in urgently providing support of various character. Key elements of the innovation: - Contact by telephone with the person concerned – patient, patient's relatives/caregiver, social worker/social nurse of the hospital and scheduling the first visit within 24 hours after hospital admission to determine the patient's condition and preliminary support plan. - Within 24 hours of discharge from hospital a nurse working for the "PROVIDER” visits the patient at home to assess the patient's condition, establish the needs of the patient and those of the caregivers, advise on assistance available and instruct the relatives. - Two additional visits and documenting the actions on the 'Verification and Support Sheet' – checking the initial assessment and verifying whether the recommendations are feasible or they need to be adjusted and whether the relatives require additional support that they were not aware of previously. - The nurse provides caregivers with practical advice during her visits. She demonstrates how to properly assist the patient, how to feed, wash, change diapers, underwear etc., depending on the patient's condition and independence. She advises on how to get the proper nursing equipment and how to adapt the room to meet the patient's needs. The nurse provides detailed information on the illness, expected developments and suggests how to re-organize the lives of co-residents and caregivers.
Who is the innovation for?
people recovering from an illness, discharged from hospital, who require the assistance of a caregiver; – people living with the patients, becoming their natural caregivers.

– Nursing entities; – local government units.
Products resulting from testing
An innovation model; – Tools for nurses – individual in–depth interview questionnaire, patient support sheet, verification and support sheet; –Tools for the patient – instructions in the form of leaflets; –Tools for the organizer – training materials for nurses.
Results achieved as a result of testing
With this innovation model seven patients benefited from comprehensive care provided at their homes. Because of the very long waiting period for this type of assistance, they were keen to participate in the trials of the new care model. When visited by the nurse, the families reported positive effects of the assistance. The care was provided at a difficult moment in their lives, and the sense of security gave them time to consider all available options and plan further actions. They found the educational material helpful and informative. The caregivers were very pleased to be able to contact the nurse by telephone.
