This innovation aims to counteract the fact that elderly dependants do not know about social and cultural events developed for them, which results in their limited social contacts and low physical activity.
How does innovation work?
The innovation helps elderly dependants be more socially and culturally active. This is achieved by activating a support network: a collaboration of local social welfare, health, education, cultural and sports institutions. The innovation encourages healthcare professionals to inform older patients about social and cultural events intended for them and to promote their active participation in such events. The 'social prescription' they issue includes information on available physical activities and events, grouped according to the 'independence index', with medical indications for participation in selected ones.
